From traditional laparotomy to three-port laparoscopic surgery to single-port laparoscopic surgery, the new treatment technology of 4K ultra-high definition laparoscopy provides patients with more diversified treatment options.

Indications

Hysterectomy and adnexectomy is suitable for those over 40 years old, whose condition is serious, and who have no desire to have more children.

Contraindications

1. Patients with uterine fibroids combined with adnexal malignant tumors, endometrial cancer stage II or above, or cervical cancer stage Ib or above should not undergo simple hysterectomy.

2. Acute pelvic inflammatory disease.

Preparation before surgery

1. When possible, pelvic angiography should be performed to confirm the diagnosis.

2. Laparoscopy, carefully observe the location and situation of venous distention, make appropriate plans, and perform surgery.

3. Same as abdominal hysterectomy.

Surgical steps

1. First open the open ligament and remove the varicose venous plexus.

2. Perform hysterectomy as usual. If the patient is younger, subtotal hysterectomy may be considered to preserve unilateral or bilateral ovaries. If you are near menopause and the ovarian veins have severe varicose veins, they should be removed together with the appendages.

Classification

Obstetrics/gynecology/gynecological surgery/abdominal surgery/benign disease surgery/pelvic varicose vein surgery

Anesthesia and positioning

Continuous epidural anesthesia is appropriate, with the patient in the supine position.

Key points to note during surgery

1. When incising the posterior leaf of the open ligament, care must be taken to avoid injuring blood vessels and causing bleeding. Do not injure the ureter.

2. The walls of the veins in the pelvic cavity are thin and easy to break. The operation must be gentle and careful. If there is bleeding, immediately ligate, cut, and re-separate the next section.

Postoperative treatment

After hysterectomy and adnexectomy, avoid excessive physical activity and moderate sexual intercourse.
